General Case Management

What is General Case Management?
The Case Managers provide services to individuals and families facing challenges and are in need of support. After enrollment everyone is evaluated and assessed to determine the level of support they will need, access to or lack of resources/means, and possible risk to themselves or others.
Case Management Services and Appointments
Case Managers assess the factors (ie. finances, availability, any desired specialty, etc.) that can influence referrals and connection to services, and we are able to tailor referrals to help meet individual/family needs.
When appropriate, the Case Managers will coordinate, support, and monitor services for individuals/families referred to more intensive or ongoing services in the community.
